Rates & Health Insurance
We accept most insurance plans, with 95% of our clients paying $0!
We have listed some information below based on our knowledge and experience. However, since you are ultimately responsible for payment, we highly recommend you call the number on the back of your insurance card and ask these specific questions.
Rates
If you are utilizing in-network insurance to pay for services, the allowed rate is determined by your insurance company. After processing the claim, they will send both you and us an "Explanation of Benefits" which tells us how much (if any) of the fee is your responsibility.
If paying at the time of service and not using insurance, our rates are:
With proof of active Medicaid enrollment: $160
With proof of active Medicaid enrollment: $110
No Show & Late Cancellation (<48 hours notice) Fee: $100
For late cancellations only, we will waive the fee if we are able to reschedule you for the same Mon-Fri week and you complete that appointment.
Payments
A valid credit/debit card on file is required and will be used for self-pay fees, amounts insurance tells us are your responsibility (i.e. copays, deductibles, and non-covered services), and in case of late cancel or no show. You may also keep a HSA/FSA card on file for eligible transactions. Any refunds issued will be less the card processing fees we have incurred.
Your Rights Under the No Surprises Billing ActPayment Policy
If your outstanding balance remains above $300 for at least 21 days, you will be blocked from booking new appointments. Your existing appointments may continue during a 30-day grace period. If the hold still applies when that period ends, we may cancel your remaining future appointments. Paying your balance down to $300 or less clears the hold.
Attendance Policy
We understand that life happens. If you miss or cancel three consecutive nutrition appointments with your clinician, they may end your current care relationship after reviewing your situation. You are still welcome at Nabi, and our team can help you connect with another clinician.
